Summary: This position is responsible for assessment, diagnosis, treatment, management, education, health promotion and care coordination for patients and families with acute and chronic health needs while working collaboratively within a multidisciplinary health team to provide comprehensive care to both well and ill patients in an outpatient setting and demonstrating a high degree of clinical expertise in working with patients with acute and chronic illnesses commonly encountered in general practice for pediatrics and adults.

Essential Duties:

Functions independently to perform age-appropriate history and physical for complex acute, critical, and chronically ill patients, which may include psychosocial, cognitive and/or personal.

Demonstrates clinical competence in direct care of patients, orders and interprets diagnostic and therapeutic tests relative to patient's age-specific needs.

Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities.

Implements interventions to support the patient to regain or maintain physiologic stability, including but not limited to serving in the first assisting role. Monitors the effectiveness of interventions. Serves as a patient advocate.

Collaborates with multidisciplinary team members by making appropriate referrals.

Facilitates staff, patient and family decision making by providing educational tools, monitoring patients/family response to education and acts as a resource in implementing education programs to improve management of patients/families.

Qualifications:

Education: Graduate of an accredited school of nursing and Graduate of an accredited Nurse Practitioner Program. Must have a Current Advanced Practice Registered Nurse (APRN) Licensure and Current Nurse Practitioner certification in specialty area and current provider card in Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS).

Experience: Minimum of 5 years of progressive clinical experience with 2 years' experience in the Nurse Practitioner role preferred; or an equivalent combination of education and experience. Previous work experience in a public health or community clinic setting preferred.
